Mr. Jake Addison Shoulders, age 25 of Lafayette, TN died Saturday, May 26, 2012 at Vanderbilt University Medical Center, Nashville, TN.

He was receded in death by his father, Philip Dale Shoulders.

He is survived by wife, Mrs. Nicki Shoulders-Lafayette, TN; two sons, Dylan and Tristan Shoulders; daughter, Khloe Shoulders; mother, Mrs. Andora Shoulders-Bethpage, TN; brother, Mr. Ryon Shoulders-Bethpage, TN; grandparents, Mr. Robbie and Mrs. Shirley Shoulders-Hartsville, TN.

Funeral services were held in the chapel of Anthony Funeral Home at 2 PM Thursday, May 31, 2012 conducted by Bro. Lonnie Meador and Pastor Susan West.

Pallbearers: Adam Storey, Blake Woodard, Zach Scott, Eric Binion, Cody Thayer, Ryon Shoulders and Jared Davenport.

Interment was in the Dixon Springs Cemetery.

Visitation with the family was held from 2 to 9 PM Wednesday, May 30, 2012 and 10 AM until service time Thursday.

In lieu of flowers, donations may be made to the Children’s Fund at Macon Bank & Trust.
