Mr. Jack Smith, age 70, of Hartsville, TN died Friday, February 1, 2013 at Vanderbilt University Medical Center, Nashville, TN.

Funeral services were held in the chapel of Anthony Funeral Home at 2 PM Tuesday, February 5, 2013 conducted by Bro. J.C. Cox. Pallbearers: Terry Gregory, Brian Satterfield, Carlos West, Jamie Woodard, Roger Gregory, Joe Welker, Tommy Thompson and Ronnie Shrum. Interment was in the Hartsville Memorial Gardens.

In lieu of flowers, donations may be made to the American Cancer Society.

He was preceded in death by: Grandson, Logan Blake Smith.

He was survived by: Wife, Mrs. Thelma Smith-Hartsville, TN; Two Sons, Mr. Roger (Tracy) Smith-Lebanon, TN, Mr. Joey (Tonya) Smith-Lafayette, TN; Daughter, Mrs. Lois (Charlie) Woodard-Lafayette, TN; Five Grandchildren, Dalton Cole Smith, Joseph & Matthew Woodard, Tyler & Brayden Smith; Great Grandchild, Kenzi Woodard; Two Brothers, Mr. Alex (Ruth) Smith-Hendersonville, TN, Mr. Jim Smith-Hartsville, TN; Sister, Mrs. Sue (Olie) Allan-Gallatin, TN.
